On Oct. 1, 2024 Clarity Healthcare began services as a designated Certified Community Behavioral Health Center (CCBHC) by Illinois Health and Family Services and Office of Medicaid Innovation (OMI). The CCBHC designation expands Clarity’s service area from Adams and Pike counties to add Brown, McDonough, Schuyler and Hancock counties.

CCBHCs are designed to provide a comprehensive range of mental health and substance use disorder services to vulnerable individuals.

Designated CCBHCs directly provide comprehensive care, with emphasis on 24-hour crisis care, evidence-based practices, care coordination with primary care and hospital partners, and integration with physical health care.

Services offered within the CCBHC model include: 

• Crisis mental health and addiction services
• Screening, assessment and diagnosis including risk assessment
• Person and family centered treatment planning, direct provision of outpatient mental health and substance use services
• Outpatient primary care screening and monitoring of key health indicators and health risk
• Targeted case management
• Psychiatric rehabilitation services
• Peer, counselor and family support services
• Intensive, community-based mental health care for members of the armed forces and veterans, particularly those in rural areas
